Purpose

Use the Header and Footer Field screens to continue to design the desired format for Interim, Cumulative or Microbiology Cumulative Reports.

Header and Footer Parameters

The header and footer parameter offer identical field parameters that you can use to define the top and bottom area of the Interim, Cumulative, and Microbiology Cumulative reports.

 

The header refers to the top area of the report. Header information usually contains the performing lab name and address, the name and address of the doctor , and patient information.

 

If you use a preprinted form with some of the header information preprinted, remove the line and column designation, if previously entered, to prevent that information from printing.

 

The footer refers to the area at the bottom of the report, below the report body. The system default does not include any footer parameters.

 

The standard default parameters are present when accessing the blank parameter set for the first time. You can modify these parameters to meet the report needs of your facility.  

 

Line and Column

 

 

Note: Each parameter displayed on the header and footer screens can be included on the report if there is an entry in both the line and column text boxes.

 

To exclude a parameter from the report or to accept the default placement of the parameter, do not place an entry in the line and column text boxes.

 

 

The line and column text boxes indicate where data should print on the report. The first line on the report header is considered Line 1.

 

When you define the line and column, remember that other parameters previously defined can be affected by placement of the current parameter.

To leave a screen in the CyberLAB system, always click the Exit button at the upper-right corner of the screen.  

Do not use the (red or black) X button at the top-right corner of the screen. This action can affect operation of the application.

Definitions and Procedures

 

Note: The headers and footers for each report appear in the order that they display on the screen.

 

However, if the same label appears in the header and footer, only the header definition appears below.

 

 

 

 

 

 

Keyboard Users:  To navigate around a screen or scroll within a selection list, use the Tab key to move down and the Tab and Shift keys to move up. Press the Enter key to select the data.

 

For a shortcut key, also known as a hot key, press the Alt key and the key corresponding to the letter that is underlined, for example, Alt + O for OK.

INTERIM REPORT

 

Note: For Interim reports, the same parameters are used for headers and footers, except one. The Continued/End label is used only in footers. See below.

Lab Name

The name of the lab that generates the report. The lab is defined in General System Parameters and, if a multisite facility, the lab is defined in Site/Test Cross Reference. In a multisite facility, the CyberLAB system retains the location where each test is ordered. The lab name of the ordering site is printed, if it is a multisite.

Enter a maximum of 30 characters.

Lab Address Line 1

The street address of the lab that generates the report. Enter a maximum of 30 characters.

Lab Address Line 2

The city and state of the lab that generates the report. Enter a maximum of 30 characters.

Lab Address Line 3

The zip code of the lab that generates the report. Enter a maximum of 30 characters.

Lab Phone Number

The phone number of the lab that generates the report. Enter a maximum of 18 characters.

Patient ID

A unique ID, such as a Medical Record Number, that is assigned to a patient in the CyberLAB system. The facility assigns the ID when the patient enters for a visit, or the ID can be transmitted from the HIS/CIS. Enter a maximum of 15 characters.

Visit ID

A unique ID that identifies a particular patient visit or encounter. The Visit ID also is known as Chart ID, Admit ID, or Admission Number. Enter a maximum of 15 characters.

Patient Name

The name of the patient which is entered in Demography in the Patient Entry submodule or transmitted from an HIS/CIS. Enter a maximum of 31 characters.

Patient Street Address

Usually a number and street name which indicates the residence of the patient. Enter a maximum of 25 characters.

Patient City/State

The city and state in which the patient resides. Enter a maximum of 23 characters.

Patient Zip Code

A nine-digit number that designates a town, village, or rural area. The American zip code has a hyphen between the fifth and sixth number. The Canadian zip code has no hyphen. Enter a maximum of 10 characters.

Patient Age

The patient's age is entered when the sample is ordered. The retention of the patient's age at the sample level ensures that the correct age of the patient appears on the clinical reports and that appropriate normal ranges are used for test results, if applicable. The Age Indicator (Years, Months or Days) is also printed when the patient age is included on the report. One blank space is placed between the age and age indicator.

Enter a maximum of six characters.

Patient Date of Birth

The date of birth of the patient is entered in Demography in Patient Entry or transmitted from an HIS/CIS. If you selected the Yes option in Miscellaneous Parameters to Include Century in Dates, the date of birth prints in a 10-character format; for example, MM/DD/YYYY. If you selected the No option in Miscellaneous Parameters, the date of birth prints in an 8-character format; for example, MM/DD/YY.

 

Enter a maximum of 8 or 10 characters.

Patient Sex

Either one of two types, male or female. A third option is unknown. Enter a maximum of one character.

Species Description

A description of the species of the patient who is an animal. Used by veterinary facilities. Species defined in Species Code Description. Enter a maximum of 20 characters.

Nursing Station/Unit

The location of the patient, as entered in Patient Demography. Referred to also as Location. Enter a maximum of four characters.

Room

The number of the room in which the patient is located. Enter a maximum of four characters.

Bed

The bed in which the patient is located. Enter a maximum of two characters.

Sample ID

A unique numeric ID assigned to each order of tests for a patient. Also referred to as the Accession Number.

Enter a maximum of nine digits.

Collection Tech

The Tech ID or initials of the person who collected the specimen. Enter a maximum of two characters.

Collection Date

The date on which collection of the specimen is entered into the CyberLAB system. The collection date and date of entry into Order Entry may not be the same. Enter a maximum of 8 or 10 characters.

Order Received Date

The date that an order is placed in the Host or CyberLAB system. Enter a maximum of 8 or 10 characters.

Printed (Reported) Date

The date on which the completed sample was reported. It is the first reported date for the sample. Enter a maximum of 8 or 10 characters.

Miscellaneous Comment

A sample-specific comment selected from the Comment Code selection list  in Patient Entry. Note: This field can be divided into five parts; if it is divided into five, see Sample Miscellaneous Info Fields 1-5 below in this table.

Enter a maximum of 30 characters.

Patient Additional Info

A demographic-specific comment about a patient. The comment is selected from the Comment Code selection list. Note: This field can be divided into five parts; if it is divided into five, see Patient Additional Info Fields 1-5 below in this table.

Enter a maximum of 45 characters.

Report Status

Indicates the status of the report: Final or Preliminary. Enter a maximum of 25 characters.

Report Doctor ID

 

(Send-To Doctor ID to be changed to Report Doctor ID.)

The Client ID of the Report doctor who receives a copy of the clinical report.

 

The Report Doctor header and footer are driven by the Yes or No setting for this parameter in Miscellaneous Parameters: Retain Original Ordering Doctor on Copy-To Reports. The Yes option keeps the original ordering doctor on the report and adds the name of the Copy-To doctor to the report.  The No option does NOT keep the original Ordering doctor on the report and replaces the name with that of the Copy-To doctor.

 

Enter a maximum of nine digits for Report Doctor ID.

Report Doctor Name

The name of the Report doctor who receives a copy of the clinical report. See Report Doctor above.

 

Enter a maximum of 41 characters.

Report Doctor In Care Of

 

(Send-To Doctor In Care Of to be changed to Report Doctor In Care Of.)

The company or hospital name with which the Report doctor who receives a copy of the clinical report is associated. See Report Doctor above.

 

Enter a maximum of 25 characters.

Report Doctor Street

 

(Send-To Doctor Street to be changed to Report Doctor Street.)

The street address of the Report doctor who receives a copy of the clinical report. See Report Doctor above.

Report Doctor City/State

 

(Send-To Doctor City/State to be changed to Report Doctor City/State.)

The city and state in which the Report doctor who receives a copy of the clinical report resides. See Report Doctor above.

Report Doctor Zip Code

The zip code of the Report doctor who receives a copy of the clinical report. See Report Doctor above.

Report Doctor Phone No.

The phone number of the Report doctor who receives a copy of the clinical report. See Report Doctor above.

Attending Doctor ID

 

 

The unique numeric ID assigned to the doctor by the facility.

Note: These doctor fields (Attending ID and Name) usually refer to the Admitting doctor. But, if the Miscellaneous parameter, Retain Original Ordering Doctor on Copy-To Reports, is set to No, then the name of the original Ordering doctor is replaced by the Copy-To doctor. This Miscellaneous parameter is a feature that is intended to enable the Attending doctor to remain on the report, if the Yes option is selected.

 

Enter a maximum of nine digits.

Attending Doctor Name

The name of the doctor assigned to the patient while he or she is in the facility. See Attending doctor ID above. Enter a maximum of 41 characters.

Page Number

The page number of the report starts with 1 and increases by one on each page of the report. Enter the label for the page; for example, PAGE: The page numbers can increase to 2 or 4 digits, as designated in Miscellaneous Parameters. Enter a maximum of four characters.

Standard Column Header

This header prints between the patient header and the body of the report for all Sample ID, except Microbiology and Cytology only. Enter a maximum of 80 characters.

Micro-Only Column Header

Note: This header prints only if all the tests in the Sample ID are micro type tests.

 

This header prints between the patient header and the body of the Microbiology Interim report. It is recommended although not required to assign the same line and column as the Standard column header because the entire header, footer and body information use common parameters.

 

Enter a maximum of 80 characters.

Cyto-Only Column Header

Note: This column header is used when only tests defined as Bethesda Cytology type are in the Sample ID.

 

The header that prints between the patient header and the body of the cytology report. It is used when cytology tests are ordered.

 

It is recommended but not required to assign the same line and column as the standard column header as the entire header, footer, and body information use common parameters.

 

Enter a maximum of 80 characters.

Lab Director Name

The name of the lab director which is specified in the General System Parameters submodule. Enter a maximum of 58 characters.

Patient Phone No.

The phone number of the patient. Enter a maximum of 18 characters.

Copy-To Provider ID

The unique numeric ID of a client or a facility to which a copy of the patient report is to be sent. Enter a maximum of nine digits.

Copy-To Provider Name

The name of a client or a facility to which a copy of the patient's report is sent. Enter a maximum of 41 characters.

Copy-To Provider In Care Of

The company or hospital name associated with the client or facility to which a copy of the patient's report is sent. Enter a maximum of 25 characters.

Copy-To Provider Street

The street address of the client or facility to which a copy of the patient's report is sent. Enter a maximum of 25 characters.

Copy-To Provider City/State

The city and state in which the client or facility that receives a copy of the patient's report resides. Enter a maximum of 25 characters.

Copy-To Provider Zip Code

The zip code of the client or facility that receives a copy of the patient's report. Enter a maximum of 10 characters.

Copy-To Provider Phone No.

The phone number of the client or facility that receives a copy of the patient's report. Enter a maximum of 19 characters.

Collection Time

The time at which the collection of the specimen is entered in the CyberLAB system. Enter a maximum of five characters.

Order Received Time

(Received Time to be changed to Order Received Time.)

The time that an order is placed in the Host or CyberLAB system. Enter a maximum of five characters.

Printed (Reported) Time

The time at which the completed sample was reported. It is the first reported time for the sample. Enter a maximum of five characters.

Preliminary Report Status

The report is preliminary when at least one test result is pending. If you use a preprinted form in which an X prints in a box to indicate the report is incomplete/preliminary, place an X in the box and enter the appropriate column and line.

 

Note:The content of the field is defined in Miscellaneous Parameters. The placement of this label is usually defined in the field labeled, Report Status. This label is used only if the user wants the preliminary in a different place than the final.

 

Enter a maximum of 25 characters.

Final Report Status

The report is final if no test results are pending. If you use a preprinted form in which an X prints in a box to indicate the report is complete/final, place an X in the box and enter the appropriate column and line.

 

Note:The content of the field is defined in Miscellaneous Parameters. The placement of this label is usually defined in the field labeled, Report Status. This label is used only if the user wants the final in a different place than the preliminary.

 

Enter a maximum of 25 characters.

Patient Type

A classification for a patient that can be used for Medical Necessity Validation rules. The types include: Inpatient, Outpatient, STAT, Emergency, Clinic, Referred and Other. Enter a maximum of one character.

Sample Type Description

An abbreviation that describes the sample priority.  The description displays at the right of Type in order entry and can be printed on worksheets. Enter a maximum of 10 characters.

Department Name

The name of the performing department in the report, if divided by department. Enter a maximum of 40 characters.

Patient Additional Info Field 1

 

The Patient Additional Info field can be divided into as many as five fields. These fields allow the user to select demographic-specific comments about a patient.

 

Enter a maximum of 1 to 45 characters for each field.

Department Director

The director of the performing department in the report. Enter a maximum of 50 characters.

Sample Misc. Info Field 1

The Sample Miscellaneous Info field can be divided into as many as five fields. These fields allow the user to select brief sample-specific comments.

 

These comments may print on a clinical report. The comment is selected from the Comment Code selection list.

Location Description Line 1

Usually refers to the location of a patient within a medical facility. The location description is maintained in  Location Maintenance in the SampleType/CyberPrint submodule in File Maintenance.

 

Enter a maximum of 20 characters on each line.

Location Description Line 2

Changed Report Label

A label that indicates a test result has been changed in the report. Enter a maximum of 50 characters.

Insurance Type

The payment method used to pay for medical services, such as Medicare, Medicaid or other insurance. Located on Demography screen. Enter a maximum of one character.

 

The Insurance Type label has replaced Bill Type.

Medicaid No.

A unique number that identifies the patient as covered by Medicaid. Enter a maximum of 15 characters.

Medicare No.

A unique number that identifies the patient as covered by Medicare. Enter a maximum of 15 characters.

Billing Company

Used for multiple billing entities. Enter a maximum of one character.

Responsible Party

The person who is liable for the medical expenses of the patient. Enter a maximum of 30 characters.

Relationship

A code that identifies the relationship of the responsible party, the person who is liable for the medical expenses of the patient. Enter a maximum of one character.

Insurance No.

A unique number for the insured, issued by the insurance company. Enter a maximum of 15 characters.

Group No.

An insurance carrier number that designates the company of the insured. Enter a maximum of 15 characters.

Insurance Company

A code for the name of the insurance company. Enter a maximum of six characters.

 

Note: This code drives Order Entry Decision Support.

Transaction Code

A transaction code that identifies the type of bill. Enter a maximum of two characters.

Sample Comments

Sample comments are sample-specific and are located in the Patient Entry submodule. The comment can be free text or selected from the Comment selection list.

 

Enter a maximum of 50 characters.

Sample Diagnosis Code

Sample-level diagnosis code in Order Entry. Enter a maximum of five characters.

First Reported Date

The first date that the test result was reported.

 

The First Reported Date/Time is significant on reports that may have multiple reported dates. This field would represent the first date/time of the report. Enter a maximum of 8 or 10 characters.

First Reported Time

The time on that date when the test result was first reported. Enter a maximum of 5 characters.

Primary Care Dr. ID (Demography)

The Client ID of the principal doctor, as entered in the Demography of the patient. Enter a maximum of nine digits.

Primary Care Dr. Name (Demography)

The name of the principal doctor, as entered in the Demography of the patient. Enter a maximum of 41 characters.

Primary Care Dr. ID (Sample)

The Client ID of the principal doctor, as entered at the sample level. Enter a maximum of nine digits.

Primary Care Dr. Name (Sample)

The name of the principal doctor for the patient as entered at the sample level. Enter a maximum of 41 characters.

Lab Received Date

The date that the specimen is received in the laboratory so that the ordered test can be performed. Enter a maximum of 8 or 10 characters.

Lab Received Time

The time that the laboratory technologist receives the specimen in the lab. Enter a maximum of five characters.

Lab Received Tech

The laboratory technologist who receives the specimen in the lab. Enter a maximum of two characters.

CLIA No.

The Clinical Laboratory Improvement Amendments (CLIA) number issued to the facility. Enter a maximum of 15 characters.

Admit Date

The date that a patient is admitted to the medical facility.

 

The admission date/time is sent from the Host system or is the date the patient's demographic information is initially entered in the CyberLAB system.

 

Enter a maximum of 8 or 10 characters.

Admit Time

The time at which the patient is admitted to the medical facility. Enter a maximum of five characters.

Discharge Date

The date that the patient was discharged from the medical facility. Enter a maximum of 8 or 10 characters.

Discharge Time

The time at which the patient was discharged from the medical facility. Enter a maximum of five characters.

Report Generation Date

The date that the report is created but not yet printed. Enter a maximum of 8 or 10 characters.

Report Generation Time

The time that the report is created but not yet printed. Enter a maximum of five characters.

Ordered Test Count

The number of tests ordered on this Sample ID on the Interim report. Enter a maximum of three characters.

Ordering Dr. ID on Copy

The Client ID of the Ordering doctor, also known as the Attending doctor. Used on the Interim report.

 

Note: If the Miscellaneous parameter, Retain Original Ordering Doctor on Copy-To Reports, is set to No, then the name of the original Ordering doctor is replaced by the Copy-To doctor. This Miscellaneous parameter is a feature that is intended to enable the Attending doctor to remain on the report, if the Yes option is selected.

Ordering Dr. Name on Copy

The name of the Ordering doctor, also known as the Attending doctor. Used on the Interim report. Enter a maximum of 41 characters.

Patient Race

A one-character code that describes the race of the patient. Enter a maximum of one character.

 

 

FOOTER PARAMETER

 

Note: For Interim reports, the same parameters are used for headers and footers, except one. The Continued/End label is used only in footers.

This field designates the location of the label. On every page but the last page, the Continued label prints. On the last page, the End label prints.

The table below contains only the Cumulative Report and Microbiology Cumulative Report headers that are not found in the Interim Report.

Definitions

 

Admitting Dr. ID

The unique numeric ID assigned to the ordering doctor by the facility. The admitting doctor is also referred to as the ordering doctor. Enter a maximum of nine digits.

Admitting Dr. Name

The name of the ordering doctor. Enter a maximum of 41 characters.

Status

Indicates the status of the report, whether it is final or preliminary. Enter a maximum of 25 characters.

Department Header

The name of the performing department on the report. Enter a maximum of 80 characters.

Department Director

The director of the performing department on the report. Enter a maximum of 50 characters.

Admitting Dr. In Care Of

The company or hospital name with which the ordering doctor is associated. Enter a maximum of 25 characters.

Admitting Dr. Street Address

The street address of the ordering doctor. Enter a maximum of 25 characters.

Admitting Dr. City/State

The city and state in which the ordering doctor is located. Enter a maximum of 25 characters.

Admitting Dr. Zip Code

The zip code where the ordering doctor is located. Enter a maximum of 10 characters.
